Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling.
After you request an appointment, you will receive an invitation to sign up on the patient portal. Registration forms will be available to you for completion online via your computer, tablet, or smart phone. Please allow 30-40 minutes to complete the forms. The more detailed and accurate your information is, the better. If the forms are not completed within 24 hours of your initial visit, you will need to reschedule your appointment.
BIOMe does not bill insurance, but this information is collected and shared when required for other entities that do bill insurance. For example, insurance information is required for referrals to specialists (like cardiologists), an imaging center (for CT scans or MRIs), and to pharmacies when medications are prescribed. With regards to prescriptions, if your information is in the EMR, the system will automatically check for approval. If a prior authorization is needed, it can be completed and transmitted to your pharmacy at the time the prescription is sent electronically. If you do not have insurance, please add “n/a” to the form.
BIOMe accepts Visa, Mastercard, American Express, and Discover cards to process membership fees. BIOMe DOES NOT accept cash, checks, gift cards, Paypal, CashApp, Zelle, Venmo, or other transaction types. Membership fees will be processed monthly. Your initial fee will be processed 12-24 hours prior to your scheduled visit. Payment information is required to confirm your appointment.
You may choose a virtual or in-person new patient visit. If you choose virtual, please make sure you are in a private area with minimal distractions while conducting your telehealth visit. Also, make sure you have a good connection and check your camera/audio prior to the visit.
